It has been historically on the board difficult to keep the terminology and understanding of the terms in the title of this post, and others, clear with frequent disagreements and misunderstandings. Here it is laid out completely by Tom Campbell in this particular post from the board. It is all logical but complex. It is not however impossible to understand.

Most are based upon posts from this board which have been modified to conform more to a lecture format as they are described.
